Kevin Friend will take charge of last-day action at the Amex.
The Leicestershire-based referee has been appointed to Albion’s game versus West Ham on Sunday.
His VAR will be Simon Hooper.
Albion fans last saw Friend in the middle when he reffed their team’s 3-0 defeat at home to Burnley.
Appointments have also been confirmed for matches which will decide the biggest issues in the Prem.
The title race will be referred by two leading officials as Anthony Taylor takes Liverpool v Wolves and Michael Oliver handles Manchester City v Aston Villa.
Their respective VARs are Stuart Attwell and Darren England.
In the relegation battle, Paul Tierney will referee Leeds’ game at Brentford while Craig Pawson has been assigned Burnley v Newcastle.
Mike Dean bows out before retirement with a trip to Stamford Bridge as Chelsea take on Watford in a game where nothing of note is at stake.
Jon Moss’s final game is Leicester v Southampton.
