Another day and another crucial mistake at a hospital run by Brighton Health Care NHS Trust.
The eyes of Britain were on the Royal Sussex County Hospital in Brighton at the weekend when a patient died after an experienced consultant gave an injection to him in the wrong place.
Now a three-year-old boy at the Royal Alexandra Children's Hospital has been given five times the intended dose of an anti-viral drug being administered because he was suspected of having meningitis.
Luckily the dose was not fatal and the mistake was realised before the error could be repeated. But it raises worrying questions about standards at these hospitals.
Two separate inquiries are now being held into what went wrong. The results must be made known as soon as possible so that the trust can improve procedures wherever necessary and reduce the risk of these tragic errors happening again.
