Brighton and Sussex University Hospital NHS Trust have apologised for shortcomings that could have lead to Indya Trevelyan's death.
The 20-month-old died in 2007 after a hospital procedure went wrong at the Royal Alex Children's Hospital, Brighton.
Trust medical director Matthew Fletcher told The Argus that although no action will be taken against any individual members of staff, an action plan has been drawn up and changes must be implemented.
He added: “Indya’s death was a tragedy and we are so sorry that it happened.
“Everyone involved in her treatment and care has been profoundly affected by what happened.
“There are no words or actions that can change this but we have done our very best to provide a detailed, clear and honest account to Indya’s family, the coroner and the authors of this independent external review.
“We have shared the review with Mr and Mrs Trevelyan.
“Along with our chief executive Duncan Selbie, I will be meeting with them next week to answer any questions they have about its findings.”
